“A resident brought an unloaded firearm into the residence from a home visit, which violates the facility's policy prohibiting storage or possession of firearms or weapons anywhere on the property. The firearm was immediately removed by the resident's designated person.”

Staff were aware of threats made by one resident toward another for several weeks prior, reported multiple times through huddles and communication logs, but were only verbally instructed to keep the residents apart without implementing adequate supervision or behavioral intervention measures.”

“Hot water temperature at bathroom sink in one living unit measured 128 degrees Fahrenheit and at the kitchen sink in another living unit measured 134.4 degrees Fahrenheit, both exceeding the maximum allowed temperature of 120 degrees Fahrenheit.”
