Additionally, an electrical outlet in the wall on the 1st floor secure dementia care unit C between the juice machine and water machine lacked a cover plate, exposing electrical components.”

“Food requiring refrigeration not stored at or below 40°F and/or frozen food not kept at or below 0°F, and/or thermometers not present in refrigerators and freezers.”

“Resident's enabler bars attached to bed were not securely attached to the bed frame and could be moved easily from side to side, creating gaps between the bars and the bed frame.”

“Unlocked, unattended, and accessible bottle of Drug Buster was found in lower cabinet in Secure Dementia Unit D with label indicating to consult physician or Poison Control Center if swallowed. Additionally, unlocked, unattended, and accessible bottle of REMEDY Essentials Spray Cleanser was found in bathroom of Unit D with warning labels. All residents in secure dementia unit D assessed as incapable of recognizing and using poisons safely.”

Additionally, 4 unlabeled, undated containers of diced tomatoes found in main kitchen walk-in refrigerator and unlabeled, undated bags of frozen chicken drumsticks and funnel cakes found in main kitchen's standing freezer.”

Additionally, the SUA electrical closet was unlocked and accessible to residents in the secured dementia care unit, containing multiple poisonous materials including air fresheners, bleach germicidal wipes, and disinfectant deodorant labeled with poison control warnings; these residents are not assessed to safely handle and identify poisons.”

“Sanitary conditions were not maintained in the kitchen: crumbs, food debris, and a brown sticky substance were noted on the lower tray of the food prep table; 3 buckets of stagnant water were located underneath the stainless steel triple sink; and 2 pieces of silverware were found on the floor under the sink.”
