“Container of grab and snack chocolate pudding and Yoplait light strawberry/banana yogurt, both opened and labeled, were observed in the Boathouse neighborhood medication cart, exposing food to contamination during medication administration.”

“Missing drain cover in bathroom shower in Cottage Place neighborhood exposed a large drain hole which presented a hazard to residents using the shower.”

“Kitchen cabinets in Cottage Place and Boathouse neighborhoods had very loose brackets causing cabinet doors to hang below the bottom of the cabinets, creating a hazard.”

“A tablet and small white round pill were observed loose inside the Boathouse medication cart, violating proper medication storage requirements.”

“Unlocked, unattended, and accessible cabinets in the kitchen and bathrooms contained poisonous materials including soap, deodorant, denture cleaner, hand sanitizer, shaving cream, and aftershave. All residents were assessed as incapable of safely recognizing and using these materials.”

“Black substance was observed inside a resident toilet bowl indicating unsanitary conditions.”

“Staff member struck a resident on the head with the resident's cane, resulting in a ½ inch laceration. The incident occurred at approximately 2:45 AM in a hallway after the resident struck the staff member first.”

“Trash dumpsters outside the home were observed overflowing with trash and later found open without lids closed, failing to keep trash in covered receptacles that prevent penetration of insects and rodents.”

“Kitchen refrigerator in Boathouse Cove household contained wrapped, unlabeled, and undated lunch meat, violating the requirement that outdated or spoiled food or dented cans may not be used.”

“A prescribed medication for Resident #2 (1 tab by mouth every 6 hours as needed) was not found in the home, indicating a failure to develop and implement procedures for safe storage, access, security, distribution and use of medications.”

“Resident #1 prescribed insulin injection was not properly documented on the Sliding Scale Coverage and Glucose Monitoring Flow Sheet on multiple dates when blood glucose was measured at greater than 200, indicating failure to follow the prescriber's orders. This is a repeated violation from 11/7/23.”

“Resident #2 and Resident #3 were admitted to the Secure Dementia Care Unit, but their initial support plans were not completed within 72 hours of admission as required.”

“Resident records containing resident names, hospice statuses, and special diets were found stored in unlocked, unattended drawers and cabinets in kitchen areas, rather than in locked containers or secured enclosed areas. This is a repeated violation from 11/7/23.”

“The electric fireplace had no protective guards in place to prevent residents from coming in contact with the heat source. The fireplace becomes very hot to the touch within 30 seconds of being turned on.”

“Direct care staff person hired on an unspecified date is providing unsupervised ADL services without documentation of completion and passing of the Department-approved direct care training course and competency test.”

“Two containers of Clorox Disinfecting Wipes and a bottle of Drug Buster Drug Disposal liquid with poison warning labels were unlocked, unattended, and accessible in the Health Center room. Not all residents, including Resident 1 and 5, have been assessed as capable of recognizing and using poisons safely.”

“Outside the door to the cottage in the courtyard, there is a piece of siding missing from the exterior wall, measuring approximately 6x24 inches.”

“On 11/07/2023 at 10:20 AM, the Silver and Black GE refrigerator had a temperature of 50°F (above the required maximum of 40°F) in the refrigerator section and 10°F in the freezer section. On 11/08/2023 at 9:50 AM and 1:21 PM, the same unit showed similar temperature violations.”
