“During overnight shifts (11:00 PM to 7:00 AM) on multiple dates, 46 residents were present in the home with no staff members certified in first aid and CPR, violating the requirement that at least one staff person per 50 residents be certified and present at all times.”

“During a fire drill, the home exceeded the maximum evacuation time of 15 minutes that was set by the certified fire safety expert.”

“During multiple fire drills (on or about dates 8/28/24, 5/23/24, 4/26/24, and 3/29/24), not all residents were evacuated from the building to the designated meeting place.”

“Resident #1's most recent medical evaluation did not include required items: body positioning and movement stimulation for residents (if appropriate), health status, and mobility assessment.”

“Resident #3's preadmission screening form does not include a determination that the needs of the resident can be met by the services provided by the home.”

“The resident-home contract for Resident #1 was not signed by the resident. The resident has a dementia diagnosis and is unable to sign; the Executive Director documented this and the resident's agreement was noted on the contract.”

“Staff Member A and Staff Member B, who do not have current diabetic training, performed blood sugar testing on Resident #2. Department-approved diabetes patient education program must be completed within the past 12 months to administer insulin injections or related services.”

“Residents #2, #3, and #4 have not been educated regarding their right to question or refuse a medication if they believe there may be a medication error. Documentation of this resident education shall be kept in the resident's file.”

“Resident #3's support plan does not document the need for an enabler bar, although an enabler bar was observed attached to the resident's bed frame. The support plan must document medical and other care services that will be made available or referrals recommended by the physician.”
