“The medication cart contained expired eye drop medications: resident #1's Timolol 0.5% opened 9/24/25 (expired after 28-30 days); resident #2's Latanoprost opened 9/24/25 (expired after 6 weeks); resident #3's Brimo/timol sol 0.2/0.5% opened 9/29/25 (expired after 28-30 days).”

“Staff member physically poked a resident in the face and verbally antagonized the resident after the resident made a derogatory comment. The staff member continued to antagonize the resident despite being told to stop touching them.”

A resident in the Secured Dementia Care Unit accessed the laundry room, ingested a laundry detergent pod, and required emergency medical intervention. The laundry room door's auto-lock function was not working at the time of the incident.”

“The exit door located in the stairwell near Resident Room 14 was sticking and required force to open. The door leads outside to the locked courtyard.”

“A sleeping hour fire drill conducted on 10/12/23 at 1:18 am had a documentation error: staff person A confirmed 5 staff participated, but 10 staff were documented on the log.”
