“Based on observation and interview, the manager failed to ensure that the premises and equipment were cleaned and disinfected. Findings include: 1.  During an environmental inspection of the facility, the Compliance Officer observed in the front and back common areas a buildup of dirt and debris. There were boxes, plastic bags, and clutter throughout the rooms. 2. During an environmental inspection of the facility, the Compliance Officer observed in the front common bathroom, toothbrushes, toothpaste, combs, brushes, and used towels on the counter. There was a thick buildup of dirt and debris along the baseboards and around the toilet. There was a black ring around the bathtub/shower area. 3. During an environmental inspection of the facility, the Compliance Officer observed in the kitchen, around the stove and counter, a greasy film and dirt buildup. 4. During an environmental inspection of the facility, the Compliance Officer observed on the back outside porch, large amount of cigarette buds and ashes on the table. 5. In an interview, E1 acknowledged the premises were not cleaned or disinfected.”
