“Based on documentation review, observation, record review, and interview, the governing authority failed to designate, in writing, a manager who had either a certificate as an assisted living facility manager issued under Arizona Revised Statutes (A.R.S.) § 36-446.04(C), or a temporary certificate as an assisted living facility manager issued under A.R.S. § 36-446.06. The deficient practice posed a risk if the assisted living facility was unable to ensure compliance with applicable Rules. Findings include: 1. A review of Department documentation revealed O1 was the manager of the facility effective October 7, 2024. 2. During the environmental inspection of the facility, the Compliance Officer observed O1's manager certificate posted with an expiration date of March 23, 2025. 3. A review of O1's personnel record revealed a hire date of June 10, 2024. 4. In an interview, E1 acknowledged the governing authority failed to designate, in writing, a manager who had either a certificate as an assisted living facility manager issued under A.R.S. § 36-446.04(C), or a temporary certificate as an assisted living facility manager issued under A.R.S. § 36-446.06 between March 24, 2025 through on-site inspection of April 2, 2025.”
“Based on record review and interview, the manager failed to ensure documentation of completion of a caregiver training program approved by the Department or the Board of Examiners for Nursing Care Institution Administrators and Assisted Living Facility Managers (NCIA Board), for one of three personnel records sampled. The deficient practice posed a risk if the individual was not qualified to provide the required services. Findings include: 1. The compliance Officer arrived at the facility at approximately 12:30 pm and was greeted at the door by E2. No other personnel members were present in the Home. 2. A review of E2's personnel record showed no caregiver certificate. 3. A review of the NCIA Board verification of caregiver training portal (https://azcg.tmutest.com/search) revealed E2 had not completed a caregiver training program after August 3, 2013. 4. In an interview, E1 acknowledged documentation was not available that showed E2 completed a caregiver training program approved by the Department or the NCIA Board.”
“Based on record review and interview, the manager failed to ensure a caregiver or an assistant caregiver is only assigned to provide the assisted living services that the caregiver or assistant caregiver has the documented skills and knowledge to perform, for two of two sampled personnel records, which posed a health and safety risk. Findings include: 1. Reviewed the personnel records for E1 (started June 2024) and E2 (started March 2025). These sampled caregivers' records contained no verified documentation that they had the skills and knowledge to care for R1's G-tube care. 2. In an interview, E1 reported that E1 was the only one providing G-tube care (food and medicine) to R1 and acknowledged that there was no verified documentation available for review of the caregivers' skills and knowledge to provide G-tube care.”
