“Based on documentation review, record review, and interview, the manager failed to ensure a personnel record for each employee included the individual's completed orientation and in-service education required by policies and procedures, for one of three personnel sampled. Findings include: 1 . A review of facility documentation revealed a policy titled "Orientation and In-Service Training." The policy stated "It is required that each employee and volunteer receives orientation before providing assisted living services to a resident...Orientation in regards to job ethics for each employee is to be provided before getting hired." 2 . A review of E1's personnel record revealed an orientation dated January 10, 2022. However, the orientation was not initialed or signed by the employee as being completed. 3 . In an interview, E1 reported E1 had an orientation completed before this one, but was unable to locate it. E1 acknowledged E1 had no orientation as required according to policies and procedures.”
