Based on record review and interview, the manager failed to ensure a caregiver documented the services provided in the resident's medical record. Findings include: 1. A review of R1's medical record revealed a service plan dated October 24, 2024. The service plan reported R1 was dependent in toileting and incontinence care would be provided every 3 to 4 hours and as needed. 2. Further review of R1's medical record revealed an activities of daily living (ADL) sheet for March 2025. However, Incontinence care was not marked as provided from March 1, 2025 to March 10, 2025. 3. In an interview, E4 reported the services were provided and unsure why they were not marked. E4 acknowledged R1's ADL sheet did not include documentation that showed incontinence care was provided.

Based on observation and interview, the manager failed to ensure hot water temperatures are maintained between 95º F and 120º F in areas of an assisted living facility used by residents. Findings include: 1 . During an environmental inspection of the facility, the Compliance Officers observed a bathroom sink accessible and used by residents. The water temperature of the sink measured 136.4º F. 2 . In an interview, E4 acknowledged the water temperature was not maintained between 95º F and 120º F in an area used by residents.
