Based on documentation review, observation, and interview, a person established, conducted, or maintained a health care institution without a current or valid license issued by the Department. The deficient practice posed a risk to the physical health and safety of a resident.  Findings include: 1. A review of department documentation revealed the facility with the same address was closed on October 10, 2025. 2. In an electronic interview, E2 revealed in an email, “This facility was previously licensed under License No. AL12488H. The certificate is attached for your reference. At present, we have 10 residents.” 3. The Compliance Officer observed ten residents when the Compliance Officer was conducting an initial inspection on February 12, 2026.  4. A review of R1’s, R2’s, R3’s, R4’s, R5’s, R6’s, R7’s, R8’s, R9’s, and R10’s medical records revealed a medication administration record (MAR) and an activity of daily living (ADL) for the month of February 2026. On the MARs and ADLs it showed services being provided and medications being administered from February 1, 2026 to February 12, 2026.  5. A review of R2’s service plan revealed the service plan was updated December 1, 2025 and a signature was signed on the manager's signature line.  6. A review of R3’s service plan revealed the service plan was created January 21, 2026 and a signature was signed on the manager's signature line.  7. A review of R4’s service plan revealed the service plan was created January 15, 2026 and a signature was signed on the manager's signature line. 8. A review of R7’s service plan revealed the service plan was updated November 11, 2025 and a signature was signed on the manager's signature line. 9. A review of R8’s service plan revealed the service plan was updated November 19, 2025 and a signature was signed on the manager's signature line. 10. A review of R9’s service plan revealed the service plan was updated December 1, 2025 and a signature was signed on the manager's signature line.  11. A review of R10’s service plan revealed the service plan was updated November 10, 2025 and a signature was signed on the manager's signature line. 12. A review of R3’s residency agreement revealed the resident was admitted in January 2026 when there was no active state license. 13. A review of R4’s residency agreement revealed the resident was admitted in January 2026 when there was no active state license.  14. In an interview, E1 reported that the license fees were not paid, and that was why they are reapplying to reopen the facility.  15. In an exit interview, the findings were reviewed with E1, and no additional information was provided.
