Based on documentation review, observation, and interview, the manager failed to ensure an assisted living facility authorized to provide directed care services provided access to an outside area that monitored or alerted employees of the egress of a resident from the facility. Findings include: 1. Documentation review revealed that the facility was licensed at the directed care level. 2. A review of the facilities policies and procedures revealed a document titled "Safety of Wandering Residents". It stated "If alarms are being used on doors and/or windows, the caregiver will check them daily for operation and security. Alarms that are triggered will be investigated immediately by the caregiver on duty." 3. During an environmental inspection, the Compliance Officers observed that the front door and back door had door alarms. However, testing both doors revealed the alarms were not turned on. 4. In an interview, E1 acknowledged that both alarms were turned off because the sound can be loud. This is a repeat deficiency from the compliance conducted on July 20, 2023.

Based on observation and interview, the manager failed to ensure that poisonous or toxic materials stored by the assisted living facility were maintained in a locked area inaccessible to residents. Findings include: 1. During an environmental inspection, the Compliance Officer observed that the cabinet doors underneath the sink were unlocked. Opening the cabinet revealed a spray can of Home Remedy Plus Stainless Steel Cleaner and Ajax with Bleach. 2. In an interview, E1 acknowledged that the cabinet doors storing cleaning chemicals were not locked.
