Based on record review and interview, the assisted living home failed to maintain a standardized emergency responder form for each resident, which included the information prescribed in subsection A of this section. The deficient practice posed a risk to the health and safety of a resident.   Findings include:   1. A review of R1's and R2’s medical records revealed no documentation of a standardized emergency responder form.   2. In an exit interview, the findings were reviewed with E1, and no additional information was provided.

In document review, the facility's policies and procedures included documentation they were last reviewed in 2020, and required updating. 2. During an interview, E1 reported new policies were added; however, acknowledged the facility's policies and procedures were not reviewed at least once every three years, and updated as needed.

The deficient practice posed a health and safety risk to residents and employees, if the employees were unable to implement the evacuation plan. Findings include: 1. In documentation review, the facility had documentation a evacuation drill was last conducted on October 1, 2023. No further documentation of a evacuation drill conducted since October 1, 2023, was provided. 2. During an interview, E1 acknowledged the documentation provided for review did not include documentation a evacuation drill was conducted since October 1, 2023.
