“Based on documentation review, record review, and interview the manager failed to ensure a caregiver's or assistant caregiver's skills and knowledge were documented and verified before the caregiver or assistant caregiver provided physical health services or behavioral health services, and according to policy and procedures, for two of two sampled caregivers. The deficient practice posed a risk if the employees were unable to meet a resident's needs. Findings include: 1. A review of facility documentation revealed a policy and procedure for how skills and knowledge would be verified and documented was not available for review at the time of inspection. 2. A review of E2's and E3's personnel records revealed E2 and E3 were hired as caregivers. However, the personnel records did not contain documentation to indicate E2's and E3's skills and knowledge were verified. 3. In an interview, E1 acknowledged E2's and E3's personnel records did not contain documentation at the time of the inspection to indicate E2's and E3's skills and knowledge were verified before E2 and E3 provided physical health services to residents, and according to policies and procedures.”
