“Based on record review, documentation review, and interview, the manager failed to ensure a personnel record for each employee included documentation of first aid training, for one of two employees sampled. The deficient practice posed a risk if an employee was unable to meet a resident's needs during an emergency. Findings include: 1. Review of E2's personnel record revealed no documentation of first aid training. No other documentation was provided to show E2 had first aid training at the time of the inspection. 2. A review of the facility's work schedule for January 2025 revealed E2 worked Monday through Friday 7am to 7pm for the entire month. 3. In an interview, E1 and E3 acknowledged the personnel record for E2 did not contain documentation to indicate E2 was trained in first aid.”
