“Based on record review, documentation review, and interview, the health care institution's chief administrative officer failed to ensure training and education related to recognizing the signs and symptoms of tuberculosis (TB) was provided annually to individuals employed by the health care institution for three of three personnel sampled. The deficient practice posed a potential illness risk to residents. Findings include: 1. A review of E1, E2, and E3's personnel records revealed no documentation of completed annual training on recognizing the signs and symptoms of TB. Based on E1, E2, and E3's dates of hire, this documentation was required. 2. A review of facility documentation revealed a policy titled "Tuberculosis (TB) Control - Tuberculosis Screening." Under the section titled Annual TB Education," it stated, "All individuals employed by the facility...will be required to complete Tuberculosis (TB) Training and Education upon hire and annually thereafter." 3. Further review of facility documentation revealed another policy titled "Orientation and In-Service Training" (for All Employees and Volunteers). The policy stated, "5. Tuberculosis (TB) Training and Education will be provided upon hire and at least every 12 months thereafter." 4. In an interview, E1 acknowledged the facility did not have a TB Training and Education program implemented; and E1, E2, and E3 had not completed the required training on recognizing the signs and symptoms of TB.”
