“Based on record review and interview, the manager failed to ensure a resident provided evidence of freedom from infectious tuberculosis before or within seven calendar days after resident's date of occupancy and as specified in R9-10-113. Findings include: 1. A review of R1's medical record revealed documentation of a negative chest X-ray. Further review revealed signs and symptoms and risk assessment documentation. There was no documentation of a tuberculin skin test or a TB blood test. Based on the resident's date of acceptance, this documentation was required. 2. In an interview, E1 acknowledged that R1 did not have proper documentation of freedom from infectious tuberculosis.”

“Based on record review and interview, the manager failed to ensure a resident had a service plan that was established, documented, and implemented that included the level of service the resident was expected to receive. Findings include: 1. A review of R1's medical record revealed a service plan dated June 20, 2025. 2. Further review of R1's service plan revealed no documentation of the level of service the resident was expected to receive. 3. In an interview, E1 acknowledged that R1's service plan did not include the level of service they were expected to receive.”
