A Residential Care Facility for the Elderly (RCFE) is a non-medical residential care home licensed by California CDSS under Health & Safety Code §1560. Residents receive assistance with daily living activities such as bathing, dressing, meals, and medication management in a home-like setting. RCFEs are not hospitals or skilled nursing facilities — they do not provide round-the-clock medical care.

LPA Audrey Jeung toured facility and grounds, including detached storage building. This one level home consists of 4 client bedrooms--3 with 2 beds and one with single bed--a staff room, 2 full bathrooms, staff room adjacent to kitchen with half bathroom, living room, dining room, and kitchen. Washer and dryer are located in attached 2-car garage. There are no accessible bodies of water or fire safety hazards observed. Carbon monoxide detector is tested and operable. There are lockable cabinets for secure storage of medications and toxins. A comfortable room temperature is maintained, and lighting is sufficient for safety. First-aid kit is maintained and complete. A Disaster and Mass Casualty Plan is posted. There are several persons who maintain criminal record clearances, including Ms. Tria. RCFE administrator certificates for Armando and Elizabeth Tria are observed--both expired in 2023. Licensee is requested to notify CCLD within 5 days of admitting a client. It is noted that facility phone number is disconnected and there is no new number. Upon admission of resident, a land line phone is REQUIRED. The following information and/or documents are requested to be updated and submitted to CCLD prior to admission of resident: - Designation of Administrative Responsibility (LIC308) - Personnel Report (LIC500) - Emergency Disaster Plan (LIC610E--9 page revision) - Proof of current liability insurance - Valid RCFE administrator certificate No deficiencies cited.

On 04/16/2024, Licensing Program Analyst (LPA) Jaime Vado conducted unannounced case management visit in regards to the future of the facility. The licensee requested LPA to come to the facility to discuss. During today's visit LPA met with the licensee and Rowena Cruz Diaz, licensee of two other homes. According to the licensee he is going to maintain the license and keep the facility vacant until further notice while Rowena goes through the application process and has already notified GGRC to vendorize this facility in order to move her residents from Camenchita's Residential Care Home #415600827 due to the landlord not wanting to extend the lease beyond May or June 2025. She is going to apply for a change of ownership for this location. Armando will remain the licensee and owner of the home and will provide a lease back agreement to Rowena when the time comes. Armando will no longer be accepting residents at this location until the change of ownership is approved under Rowena. LPA advised them to work with Sacramento and the Centralized Applications Branch and to not move residents at this. No citations issued. Report reviewed with Armando.

On 03/19/2024, Licensing Program Analyst (LPA) Jaime Vado conducted an unannounced required 1 year annual inspection visit. LPA met with administrator Armando Tria and explained the purpose of today's visit. This is a one level facility. According to Armando the facility is vacant and there are no resident's as of last year. LPA conducted the inspection and observed no residents in care. Resident files are still on site as well as staff. The facility is in good condition, all utilities connected, food supplies in place, and maintained ready for residents to move in should he accept more. There are no medications or belongings of any residents in the facility. He says he is still interested in keeping his license at this time and will reach back out to LPA in April regarding the future of the facility and the license. He says he intends to keep it for now. The following updated items are requested to be sent to the Department: • LIC610E Emergency Disaster Plan • LIC 308 Designation of Administrative Responsibility • LIC 500 Personnel Report • Updated administrator certificate • LIC9020 Client Roster • Certificate of Liability Insurance • Proof of control of property No citations issued.
