On 4/28/2025 at 10:40 AM, Licensing Program Analyst (LPA) A. Gomez arrived unannounced to conduct 1-Year Annual Required inspection. LPA met with Administrator, Pamela Chan and explained the purpose of the visit. The facility’s fire clearance was approved for 6 non-ambulatory of which 1 may be bedridden and a hospice waiver for 6. LPA toured facility with Pamela Chan including but not limited to bedrooms, bathrooms, kitchen, common area and backyard. The facility consists of 7 total bedrooms which 6 bedrooms are occupied by the residents and 1 bedroom is occupied by staff. All outdoor and indoor passageways are kept free of obstruction. The pool is locked and secured with a gate. A comfortable temperature is maintained at 71 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. The hot water temperature in the residents’ shared bathroom was measured at 105.7 degrees Fahrenheit. Residents’ bathrooms are equipped with grab bars and non-skid mats. There is a minimum of one week supply of nonperishable and 2-day of perishable foods. Centrally stored medication and sharps were locked and inaccessible to residents. Smoke detectors and carbon monoxide detector were in operating condition during visit. Fire extinguisher was last serviced on 12/20/2024. Emergency Disaster Plan was last posted on 5/25/2024. First aid kit was observed to be complete. Emergency disaster drill was last conducted on 4/5/2025. At 10:45AM, LPA reviewed 5 of 5 residents records. At 9:30AM, LPA reviewed 3 staff records and 3 of 3 have current first aid training and associated to the facility. No deficiencies cited during visit. Exit interview conducted and a copy of this report provided.

Inspectors conducted an unannounced infection control inspection on August 10, 2022 and found the facility properly equipped with hand-washing stations, sanitizer, and disinfection protocols for common areas. Staff were wearing appropriate protective equipment, health screenings and TB tests were current for all reviewed staff records, and the facility had adequate food supplies and a clear visitor policy in place. On 8/10/2022 at 12:26 PM, Licensing Program Analysts (LPAs) L. Francisco and P. Watson arrived unannounced to conduct Infection Control Inspection. Upon arrival, LPAs were greeted by Care Staff, Arlene Dela Paz. Administrator, Pamela Chan later arrived at 12:30 PM During the Infection Control Inspection, LPAs toured facility with Care Staff and Administrator including but not limited to front entrance, screening station, hand washing stations, bedrooms, common areas, kitchen and backyard. Facility has a sufficient 2-day perishable and one week non-perishable food supply. Visitors policy is posted on the front entrance. There is one central entry point for universal screening for staff, residents and visitors. A sign-in policy, thermometer and hand sanitizer were observed at screening station. Cough/sneeze etiquette, social distancing and hand washing posters were observed. Common touched surfaces are disinfected at least once daily. Bathrooms are equipped with liquid soap, paper towel and trash bin with touchless lids. Facility staff were observed to be wearing proper PPE. PPEs maintained at central location and easily accessible for staff. At 12:50 PM, LPAs reviewed 3 staff records and 3 of 3 have health screening and TB test results on file. Facility has a mitigation plan and maintains record of routine screening for residents and staff. No deficiency cited during visit. Exit interview conducted with Administrator and a copy of report provided.
