On 02/04/2026 at 10:15 AM, Licensing Program Analyst (LPA) Ardalan Gharachorloo arrived unannounced to conduct 1-Year Annual Required inspection. Administrator Shwe Pyae arrived at 11:20 AM. LPA met with Administrator, Shwe Pyae at 11:20 AM and explained the purpose of the visit. LPA toured facility including but not limited to bedrooms, bathrooms, kitchen, common area and backyard. All outdoor and indoor passageways are kept free of obstruction. There are no bodies of water observed. A comfortable temperature is maintained at 69 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. The hot water temperature in the residents’ shared bathroom was measured at 106.6 degrees Fahrenheit. Residents’ bathrooms are equipped with grab bars and non-skid mats. There is a minimum of one week supply of non-perishable and 2 day of perishable foods. Centrally stored medication and sharps were locked and inaccessible to residents. Smoke detectors and carbon monoxide detectors were in operating condition during visit. Fire extinguisher was last serviced on 12/30/2025. Emergency Disaster Plan was last posted on 02/04/2026. First aid kit was observed to be complete. Emergency disaster drill was last conducted on 12/30/2025. LPA reviewed 4 residents records and 3 staff records; all were complete. LPA also reviewed a sample of resident’s medications. The Updated copies of the following documents reviewed during the visit: LIC 500 Personnel Report, LIC 610E Emergency Disaster Plan, Liability Insurance, and Current Administrator’s Certificate. No deficiencies cited during visit. Exit interview conducted and a copy of this report provided.

On 01/29/2025 at 11:40 AM, Licensing Program Analysts (LPAs) P. Manalo and L. Fontanilla arrived announced to conduct Component lll with Licensee/applicant Edward Labrador. LPAs went over with Licensee/ applicant Component lll Power point presentation. LPAs provided applicant with CCLD and LPA contact information. A copy of this report was provided to applicant.

Licensing Program Analyst (LPA) Luisa Fontanilla arrived unannounced at around 10am and met with staff Jimmy Venoza. LPA explained to Venoza the purpose of the visit. The Administrator arrived at the facility at 10:18 am. The facility reported that a bedridden resident on hospice was admitted to the facility on 5/11/2025. The facility does not have a bedridden fire clearance. During the visit, LPA explained to the Administrator the difference between hospice waiver and fire clearance. Deficiency is cited per Title 22 California Code of Regulations (refer to Lic 809D). Civil penalty of $500 is being assessed for today's visit. Exit interview was conducted with the Administrator. Appeal Rights and a copy of this report were provided.
