What is an RCFE with Memory Care?

A Residential Care Facility for the Elderly (RCFE) is a non-medical residential care home licensed by California CDSS under Health & Safety Code §1560. During an annual inspection LPA observed that Judy Basa administrator certificate was expired. LPA K. Nguyen explained and provided education of administrator qualification and duties. Deficiencies are being cited in violation of Title 22 California Code of Regulations. Failure to submit proof of corrections (POC's) by plan of correction due dates along with the LIC9098 Proof of Correction may result in civil penalties. Exit interview conducted. Appeal Rights, LIC9098 Proof of Correction form, and copy of this report provided to Licensee via email at the conclusion of interview.

On 03/09/22 at 11: 13 am Licensing Program Analysts (LPA) J. Clancy-Czuleger and K. Nguyen arrived unannounced to conduct infection control inspection LPAs meet with Judy Taloma-Basa and explained the purpose of the visit. During the Infection Control Inspection, LPAs toured facility including but not limited to front entrance, screening station, hand washing stations, bedrooms, common areas, kitchen, and backyard. Facility has a sufficient 2-day perishable and one week non-perishable food supply. There is one central entry point for universal screening for staff, residents, and visitors. A sign-in policy, thermometer and hand sanitizer were observed at screening station. Hand washing posters were observed. Facility staff were observed to be wearing proper PPE. Facility has a 30-day supply of PPEs maintained at central location and easily accessible for staff. The facility has a mitigation plan. The following deficiency was observed during the visit: Water temperature was measured at 125.1 F. Chemicals were out and accessible to residents Staff 1 did not have finger print clearance Staff 2 was not associated to the facility Exit interview conducted. Appeal Rights and a copy of this report provided.

Type ACCR §87303(e)(3)

(e) Water supplies and plumbing fixtures shall be maintained as follows: (3) Taps delivering water at 125 degree F (52 degrees C) or above shall be prominently identified by warning signs.

Based on observation, the licensee did not comply with the section cited above, water tempature was measured at 125.1 F which poses an immediate health, safety or personal rights risk to persons in care. POC Due Date: 03/16/2022 Plan of Correction 1 2 3 4 By POC date licensee will submit photo proof that the water heater was adjusted so the water is between 105 and 120 degrees

(a) Disinfectants, cleaning solutions, poisons, firearms and other items which could pose a danger if readily available to clients shall be stored where inaccessible to clients.

Based on observation, the licensee did not comply with the section cited above by having laundry detergent and chemical cleaners accessible in a bathroom which poses an immediate health, safety or personal rights risk to persons in care. On 3/4/2025 at 2:15 PM, Licensing Program Analyst (LPA) Ardalan Gharachorloo arrived unannounced to conduct 1-Year Annual Required inspection. LPA met with Licensee,Judy Basa and explained the purpose of the visit. LPA toured facility including but not limited to bedrooms, bathrooms, kitchen, common area and backyard. All outdoor and indoor passageways are kept free of obstruction. There are currently no residents in care and there are no staff working at the facility. There are no bodies of water observed. The hot water temperature in the residents’ shared bathroom was measured at 112 degrees Fahrenheit. Residents’ bathrooms are equipped with grab bars and non-skid mats. Smoke detectors and carbon monoxide detectors were in operating condition during visit. Fire extinguisher was last serviced on 12/25/2024. Emergency Disaster Plan was last posted on 12/02/2024. First aid kit was observed to be complete. Emergency disaster drill was last conducted on 06/01/2024. No deficiencies cited during visit. Exit interview conducted and a copy of this report provided.

On 02/29/2024 at 1:00 PM, Licensing Program Analyst (LPA) K. Nguyen arrived unannounced to conduct 1-Year Annual Required inspection. LPA met with Administrator, Judy Basa and explained the purpose of the visit. LPA toured facility with Judy including but not limited to bedrooms, bathrooms, kitchen, common area and backyard. The facility consists of 6 total bedrooms which 5 bedrooms are occupied by the residents and 1 bedroom is occupied by staff. All outdoor and indoor passageways are kept free of obstruction. There are no bodies of water observed. A comfortable temperature is maintained at 74 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. The hot water temperature in the residents’ shared bathroom was measured at 115 degrees Fahrenheit. Residents’ bathrooms are equipped with grab bars and non-skid mats. There is a minimum of one week supply of nonperishable and 2-day of perishable foods. Centrally stored medication and sharps were locked and inaccessible to residents. Due to limited time LPA will return at a later time to complete the annual inspection.
