On 04/07/2026 at 9:45 AM, Licensing Program Analyst (LPA) Ardalan Gharachorloo arrived unannounced to conduct 1-Year Annual Required inspection. LPA met with Administrator, Olivia Rusu and explained the purpose of the visit. LPA toured facility including but not limited to bedrooms, bathrooms, kitchen, common area and backyard. All outdoor and indoor passageways are kept free of obstruction. There are no bodies of water observed. A comfortable temperature is maintained at 71 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. The hot water temperature in the residents’ shared bathroom was measured at 108.8 degrees Fahrenheit. Residents’ bathrooms are equipped with grab bars and non-skid mats. There is a minimum of one week supply of non-perishable and 2 day of perishable foods. Centrally stored medication and sharps were locked and inaccessible to residents. Smoke detectors and carbon monoxide detectors were in operating condition during visit. Fire extinguisher was last serviced on 03/12/2026 Emergency Disaster Plan was last posted on 01/26/2026. First aid kit was observed to be complete. Emergency disaster drill was last conducted on 04/02/2026 LPA reviewed 5 residents records and 5 staff records; all were complete. LPA also reviewed a sample of resident’s medications. The following documents were reviewed during the visit: LIC 500 Personnel Report LIC 610E Emergency Disaster Plan, Liability Insurance and Current Administrator’s Certificate. No deficiencies cited during visit. Exit interview conducted and a copy of this report provided.

Fire extinguisher was last serviced on 03/06/2025. Emergency Disaster Plan was last posted on 04/12/2024. First aid kit was observed to be complete. Emergency disaster drill was last conducted on 02/01/2025. LPA reviewed 6 residents records and 5 staff records; all were complete. LPA also reviewed a sample of resident’s medications. On 4/22/2022 starting at 11:35 a.m., Licensing Program Analysts (LPAs) C. Lin and K. Nguyen arrived unannounced to conduct Infection Control Inspection. LPAs met with Administrator Olivia Rusu and disclosed the purpose of the visit. Upon entry, LPAs' temperature was checked by the administrator, asked to fill out Covid-19 questionnaire, and requested to wash hands. LPAs toured facility including but not limited to front entrance, screening station, hand washing stations, bedrooms, bathrooms, kitchen, common areas, and outdoor areas. There is one central entry point for universal screening for staff, residents, and visitors. A sign-in policy, thermometer and hand sanitizer were observed at screening station. Cough/sneeze etiquette and hand washing posters were observed. Facility staff were observed to be wearing proper PPE. Facility has a sufficient 2-day perishable and one-week non-perishable food supply. Facility has a 30-day supply of PPEs maintained at central location and easily accessible for staff. Facility has Mitigation Plan, Emergency Disaster Plan, and maintains record of routine screening for residents, staff, and visitors. No deficiency cited during visit. Exit interview conducted with Administrator, and a copy of this report provided.

On 04/12/2024 at 9:30 AM, Licensing Program Analyst (LPA) K. Nguyen arrived unannounced to conduct 1-Year Annual Required inspection. LPA met with facility Administrator, Olivia Rusu and explained the purpose of the visit. LPA toured facility with Olivia including but not limited to bedrooms, bathrooms, kitchen, common area and backyard. The facility consists of 8 total bedrooms which 6 bedrooms are occupied by the residents and 2 bedroom is occupied by staff. All outdoor and indoor passageways are kept free of obstruction. There are no bodies of water observed. A comfortable temperature is maintained at 70 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. The hot water temperature in the residents’ shared bathroom was measured at 113 degrees Fahrenheit. Residents’ bathrooms are equipped with grab bars and non-skid mats. There is a minimum of one week supply of nonperishable and 2-day of perishable foods. Centrally stored medication and sharps were locked and inaccessible to residents. Smoke detectors and carbon monoxide detectors were in operating condition during visit. Fire extinguisher was last serviced on 3/1/2024. Emergency Disaster Plan was last posted on 4/12/2023. First aid kit was observed to be complete. Emergency disaster drill was last conducted on 01/10/2024. LPA reviewed 6 residents records. LPA reviewed 4 staff records and 4 of 4 have current first aid training and associated to the facility. LPA reviewed a sample of resident’s medications. No deficiencies cited during visit. Exit interview conducted and a copy of this report provided.
